Planning for the Best Outcomes
Transition services are those services and activities provided to students that specifically
help your child make a successful transition to post-secondary education, employment

• Social Security Income-1-800-772-1213. Social Security Income (SSI) is
available for children with a disability that “results marked and several functioning
limitations.” These benefits are based on guardian income until age 18 when the person’s

• MHMR-for placement on the CLASS (Community Living Assistance and
Support Services), HCS (Home and Community-based Services) and TxHML (Texas
Home and Living) waiting lists. MHMR Medicaid waivers provide financial support to
people with disabilities to support independence. To place your child on the lists you only

• Guardianship and Alternatives-In Texas, a person becomes a legal adult at age 18
and is responsible for making his or her own decisions. The law requires that
organizations honor the demands of all adults that are their own guardians. In some cases,
this leads to serious medical and/or financial outcomes.

Skills for Success
• Problem Solving-Provide opportunities for your child to assume responsibility for
learning new skills and taking on new responsibilities.
• Goal Setting-Coach your child in setting and committing to personal and
academic goals. Help him or her identify steps to achieve goals, solve problems as they
arise, and how to gain support when needed. Also, guide your child to begin thinking of
goals for after high school.
